Abstract

According to the World Health Organization, depression is the fourth largest health problem among adolescents. However, only a minority seek and receive help. Internet-based treatments increase accessibility and may lower thresholds for seeking and receiving help. Research on internet-based treatments for adolescent depression is scarce but show promising results. The overall aim of this thesis is to quantitatively and qualitatively explore efficacy, experiences and processes of a novel internet-based psychodynamic treatment (IPDT) targeting adolescent depression.

Study I evaluates the efficacy of IPDT in a sample of depressed adolescents, ages 15-18 (n = 76). Primary outcome was depressive symptomatology and was measured weekly during treatment. The results suggest that participants randomized to 8 weeks of IPDT had significantly better outcomes on the primary outcome measure of depression compared to participants who were randomized to a supportive control group (d = 0.82). This was also true for all of the secondary outcome measures (a second measure for depression, emotion regulation, anxiety and self-compassion) all favoring IPDT with medium to large effect sizes (d = 0.65–0.97).

Study II will investigate how participants experienced the therapeutic relationship and process of change in IPDT. A total of 18 interviews has been executed focusing on the young peoples’ experiences in treatment. Interviews will be analyzed using thematic analysis.

Study III will investigate whether therapeutic alliance predicts depressive symptomatology in IPDT and in internet-based cognitive behavioral therapy (ICBT). Participants (n = 272) are 15-19 years and randomized to 10 weeks of IPDT or ICBT. Alliance is rated weekly by both therapists and participants using the Session Alliance Inventory (SAI). Depressive symptoms are rated weekly by participants. How and if the therapeutic alliance is related to outcome will be investigated using linear mixed effects modelling. The alliance ratings will be investigated both as a within and between person predictor.
